Instinct vs Science

About two months ago, I wrote a little screed which got me quoted in a newspaper that “I’m fat, I know I’m fat, and I don’t care that you don’t like it”.  The point was that people in talk radio (and some politicians) are feeding the notion that fat people are what are causing health care costs to go up.  The most blatant example of this was Michael Savage advocating that “fat people” be denied  health care insurance by government edict (Mr Conservative?).

This attitude also is widely held in the medical community despite the fact that the science and data says otherwise.  Having just been hospitalized for fainting and immediately being put on high blood pressure medication and cholesterol lowering medications while I was literally strapped in a bed and unable to have a rational discussion, I feel ever more strongly now about this subject.

The “gold standard” for estimating risk of heart disease and stroke is the Framingham Risk Score.   I’m 53, not diabetic, have never smoked, not African American, no family history of premature death due to heart attacks,  my total cholesterol is under 200, my systolic blood pressure is below 140 without taking any medications.   My 10 year risk of having a heart attack is about 5% (that’s 1/2 of 1% per year – most of which is probably when I’m 60+).  That makes my risk of having a stroke or a heart attack significantly less than the average person my age – but people see a 300 pound man and their instinct says “high heart attack risk” when in fact that’s not based in fact.  Add to this that I was on a heart monitor for 4 days and didn’t have so much as a flutter and the sonogram showed my heart being normal in the way it  beats and its size (no early signs of congestive heart failure) and no heart murmer or other indication of heart disease or valve damage.

So now I’m still on a blood pressure medication to lower my systolic pressure from the 140 range to the 120 range, despite the fact that if you look at the Framingham scale that 145 untreated and 120 treated have identical risk profiles for heart attack and stroke.  So maybe you’re thinking, well it can’t hurt anything to take it….   One of the side effects of lisinipril/hctx is that it can cause symptoms of low blood pressure – fainting and dizziness – which is the reason I wound up in the hospital in the first place – in addition to causing weight GAIN.

You’ll notice that weight and BMI (body mass index) are not factors  – there is a reason for that – BMI is not an indicator of heart attack/stroke risk – in fact the highest risk is in the people who have very low BMIs.  To have a low BMI, you have to have very little muscle tissue which means you are highly inactive and a good chance you’re a smoker.  People who are moderately obese have a lower risk than those of normal weight.    The real measurement of risk is comparing the size of your waist to the size of your hips, which nobody has measured yet.

So looking to the future when the Health Care Czar is empowered to make rules based on politics, not science – if I make the above case to my doctor and he insists that I continue to take this medication despite all of the scientific data that says he’s wrong and I become “non compliant” with my doctor’s “orders”, what will the government do next?

Then again, maybe it is just a wedding cake.

Pelosi: Climate of Political Violence

The Speaker of the House Nancy Pelosi made a curious statement:

“I have concerns about some of the language that is being used because I saw … I saw this myself in the late ’70s in San Francisco,” Pelosi said, choking up and with tears forming in her eyes. “This kind of rhetoric is just, is really frightening and it created a climate in which we, violence took place and … I wish that we would all, again, curb our enthusiasm in some of the statements that are made.”

Overlooking for the moment how the world would perceive the leader of the legislative branch on the verge of tears, what exactly is she referring to – what happened in the late 1970s in San Francisco?

That was way after MLK Jr and the race riots, so that isn’t it…  The logical reference is to the murder of San Francisco mayor Masconi and gay supervisor Harvey Milk (Wikipedia article) in 1978.

Dan White – the man who killed the mayor and Milk – was a Democratic supervisor who resigned his position because of rampant corruption in city government, and financial problems due to not being able to keep his job as a fireman and sit on the Supervisor’s board. 

I don’t see any real connections between that incident and anything happening today.  Maybe Nancy is crying because she is worried about the death threats Glenn Beck is getting every day?

Seems unlikely.    There are lots of things she could do to restore civility to the Democratic process like actually meet with Republicans when legislation is being written, not trying to stifle dissent by making more rules of what House members can’t say on the floor, calling off the pit bulls at Huffington Post and maybe asking President Obama to release his long form birth certificate to get that issue behind us.

Seizing the moment

Over the past few days, Tom Taylor has been reporting recurring rumors that Bonneville Broadcasting (which is owned by the Mormon Church) may be  in discussions to acquire the former ABC radio stations from Citadel.   Citadel has been in financial limbo for about 6 months now, having failed to keep its obligations on its credit lines.  Their stock was delisted and is close to worthless.

For those of us over 50, the list of radio stations that were part of ABC radio is a who’s who of major market AM radio:

  • WABC/WPLJ-FM – New York
  • KABC/KLOS-FM – Los Angeles
  • WLS AM/FM – Chicago
  • KGO / KSFO – San Francisco
  • WBAP /  Baltimore
  • WMAL /  Washington DC
  • WJR / Detroit

Curiously, not a single former ABC Radio O&O station carries the show of  Glenn Beck.   I have to believe there is a reason for that.

Despite the pressing urgency to organize effective opposition to the looting of the country’s treasury, the Republican Party is showing no signs of competency and some leaders appear to just want to be sure they get their share of the loot.

The “family oriented” Conservative message of the Mormon Church probably has never had a more receptive audience among the American public than right now.    Owning these stations would give them a very big platform on which to influence public opinion.

New York Times is reporting that Russia has had two attack submarines  hovering off the East Coast of the United States. 

An attack submarine is used primarily to attack other vessels – other submarines or surface ships – it isn’t the type used to launch nuclear weapons.

One of the two has now headed toward Cuba.

Unidentified sources “leaked” this to the NY Times.

What was the purpose of the leak, and who is testing whom?

Communitarian in Chief

The brother of White House chief of staff Rahm Emanual is named Ezekiel J. Emanuel. Ezekiel is a medical doctor, who recently has worked in the Office of Bioethics at the National Institute of Hearlth, which focuses on issues like when medical care should be withdrawn and the patient “allowed” to die.

Dr Emanuel is a self-described Communitarian. What is a Commitarian and what do they believe?

Probably the short handed explanation is the quote from Star Trek: “Some times the needs of the many outweigh the needs of the few”, or more to the point – individual rights must give way to the needs of the collective.

Dr Emanuel is a key advisor to President Obama on the proposed National Health Care policy.

Dr Emanual’s thoughts are written down for posterity in The Ends of Human Life: Medical Ethics in a Liberal Polity. Cambridge: Harvard University Press, 1991.

This is definitely a person you want to keep an eye on.

The other side of Professor Gates

Let’s start from the premise that the people behind Barack Obama are very smart and wiley and highly motivated to transform this country into a totalitarian socialist paradise – and they think three steps ahead of their adversaries (four steps ahead of John McCain) and are ruthless.  They wil win at all costs. 

Let’s further speculate that what Professor Gates did was planned well in advance, specifically to provoke the current issue and controversy.

For those of you who are reflexively jumping to the conclusion that Professor Gates is stupid and acted recklessly and trying to stir things up…  consider for a second your own future.

You’re in your house a year from now.  A uniformed man knocks on your door and says “Hello?  I’m Sergeant Green from the Green Energy Audit Committee, and I’m here to measure your carbon outputs so we can calculate your Carbon Energy Tax”.

When you don’t come to the door, Sergeant Green announces “Mr Smith, I know you’re in there.  Just because I used to work for ACORN doesn’t mean you can ignore me.   This is a real uniform.  Please come to the door with your photo ID or I may have to call for assistance to make you comply with my lawful request.”

“Mr Smith.   We know from your gun registration records that you own a gun.   For office safety, I must now ask you to come out of your house with your hands above your head while we search your house and secure your guns for your safety.  I”m sure you can understand the reason for that.”

“I’m in my own house, I have done nothing wrong, I am not a criminal, you have no right to come into my house.  Get off my property- now”.

“Mr Smith, I’m sorry that you have resisted my lawful request.  I guess that leaves my no alternative other than to use force to comply with my lawful request. ”

Eating your seed corn

One perpetual problem faced by “do gooder” relief agencies happens when they try to help desperately poor countries deal with famine. The long term solution is to teach farmers how to grow crops, and deal with finding water, controlling pests, fertilizing and the like.

The problem is that when they give the farmers the seed corn in the spring, and return back in the fall to see how things went, they find a starving village and an empty field. The farmer will tell the aid workers they decided to eat the corn rather than plant it. [so seed corn is now treated with chemicals to make it inedible]

President Obama – and his predecessors Joseph Stalin and Mao – also did not understand this lesson and made the same mistake. If you encourage the farmer, help him nurture the crop and allow him to keep the bounty of his crop, the one seed returns 100x what was planted. If you steal the farmer’s crop in the fall to spread the wealth around to people who sat on their donkey all summer, next year he’ll eat the seed corn, and then demand you feed him and take care of him. When there is no crop left to harvest, millions of people die from starvation.

Unless of course that was the plan all along.

Sears Willis Tower to “Go Green”

Chicago’s Sears Tower claims it will reduce its consumption of electricity by 80%, and have the goal of becoming net-zero user of the power grid.  The London based Willis Insurance company will rename the Sears Tower to be Willis Tower next month.

How will technology save all this energy?   Well, they’re going to replace all of the windows in the building.  Okay, that’s useful.  I hope the new windows aren’t heavier than the ones they are replacing.   More efficient elevators and lighting control systems.     A few hundred square feet of solar panels to heat water for the 104 story building    They might try putting some wind turbines on the roof, but aren’t sure if that will work.

The description of the building says it is has a 150 Mwh power distribution system.   That’s 150,000 kw of power.   That’s a lot of power to get from 4 wind turbines.    How much energy is that?   Another stimulus funded green energy project on the south side of Chicago covering 46 acres in the former Pullman industrial site is “hoped” to generate a maximum of 10 Mwh of power.   Of course, that would only be when the sun is shining and there are no clouds.  

The building will somehow be powered by green energy… (like buying green energy offset credits from T Boon Pickens in Texas perhaps?)

Another part of the plan is to build a new hotel next door – Chicago currently has a glut of unused hotel rooms, but that could change when President Obama moves the U.S. Capital to Chicago.

A few other things to ask…  

  • Is the 80% figure for only the building or does it include the energy used by the tenants in the building?
  • How is the building heated?   Will the other changes change the heating and air conditioning load?  [answers]

Metro Rail Crash kills 9

Rather than tack more onto the post about the Air France crash, the Washington Post is now reporting what I speculated on almost verbatim:

6 killed today as one Metro train plows into another that is stopped in a station.   Media reports are blaming a failure of the signaling system, rather than the operator not noticing the impending crash and going into emergency stop.

Just a guess, but the most likely cause of this type of accident is when an engineer overshoots a station, then backs up the train on manual.   That puts the lead train into a “block” that is supposed to be empty.  The following train doesn’t know that because they already went past the green “all clear” signal.   The witness reports all agree that the moving train did not apply brakes at all prior to the crash.   I’ve ridden the Metro, and its cab control is very thorough – the only thing the operator does is to close the doors.  The train controls the acceleration and braking.   It’s very plausible that the operator could have been playing video games on her cell phone and letting the train make all the decisions.

A detail as you look at the pictures and read the accounts – you’ll see 4 tracks, with the Metro train on the inner track.   The outside tracks are NOT metro – they are the Amtrak line that runs into Union Station.   That probably greatly complicated the crash scene and the risk that an Amtrak train could have plowed into the wreckage – fortunately that didn’t happen.

What the Washington Post reported:

“The trains in yesterday’s crash were supposed to be in automatic operation, which means the operators would have been relying on the computerized system to run the trains. The only function required of a train operator during automatic operation is to close the doors after a station stop. Some safety experts said operators can “zone out” during computerized operation because they don’t have to pay as close attention as when they manually run trains.”

Already the cries are coming out for more funding for more technology to fix this system that had two fail safe systems to stop the trains – three if you count the human.

It gets worse… and further supports my theory.   There was a bizarre crash on the same Red Line in 2004 involving the same 1000 series (original 1974) cars, with the “anti-climbing” protection failing to work.  The NTSB Report can be read here.   

The determination was that in this case, the lead train was put into manual mode (it was an empty train).    The following train was in the station  unloading passengers.   The first train rolled backwards for close to half a mile down a hill until it hit the stopped train going 36 mph.  The driver had not had much sleep and probably didn’t notice the train was rolling backwards in the dark tunnel.

The operator of the empty train noticed the train starting to roll backwards.  He tried to stop the rolling by applying power (like riding the clutch on a car on a hill).   But on the older trains that doesn’t stop the train – in fact it can make it roll back faster.   The correct action is to *apply the brakes*, and the train will stop.   When the train didn’t stop, he didn’t know what to do.   The management didn’t know that in manual mode on the old trains the software wouldn’t stop a rollback, so there was no training for this situation.

That’s not what happened yesterday – the area of the crash was a downgrade – there is a curve to the right, with a fence separating the Metro from the Amtrak trains, which does somewhat limit the visibility around the gentle curve, but the top of the train is visible above the fence. 

Another interesting detail from the 2004 crash.  Non-revenue trains HAVE to be run in manual mode, since they are not in the schedule and you don’t want them automatically stopping in the stations and opening the doors –  and have people getting on the train that is just going to the storage yard.

One account says the lead train today had been stopped for 2 or 3 minutes because of “electrical problems”.  I will almost guarantee you that the first train was in manual mode trying to override the safety systems – and the train being in manual mode made it “invisible” to the train the crashed into it.   Before a train is put into manual mode, the “central control” operations center must notify other trains on the line to come to an immediate stop or operate manually at reduced speeds, or you can get exactly this type of outcome.

One curious report (hard to know if it was accurate) was from a passenger on the moving train… said there was a thump…. and she texted a message on her phone, and then the big crash happened.   That would tend to suggest that the following train’s safety system put the train in emergency stop, and then either the operator overrode on manual and/or the train was drifting downhill.     The 1000 series cars do not have data recorders on them, and the operator is dead, so it may take a bit to find out what happened.

A few more random thoughts – the fence and the right hand curve make it more difficult to see if the stopped train was in the south or north bound tracks.   Maybe she assumed the train was northbound.

The center position of the speed control is freewheeling – the train neither applies power nor applies the brakes.   Since that area has a significant grade heading down, perhaps the train protection system isn’t designed to apply brakes if there is no power being applied – and just like the 2004 accident, the train rolled due to gravity and an inattentive operator….

NTSB’s preliminary analysis says that – at the point of the collision – the moving train was on automatic, train protection was turned on, the emergency stop was depressed, and there is evidence on the wheels and tracks of an emergency stop.

The 2004 accident report contains extensive data of when each train passed each sensor and at what speed, so the NTSB claim that there will be no data about the moving train (because it doesn’t have on-board data recorders) is in error.   They also stated that the NTSB rejected the response that the Metro would not retrofit the 1000 series cars is factually wrong – the document (above) published by the NTSB clearly says “answer accepted” on both items.   Let the politically motivated lying and finger pointing  begin.

Tests yesterday revealed that one section of track failed to detect the presence of a shunt (the shunt creates a bridge between the two rails just as the axle of a rail car does – which is one way the system knows a train is present.   An earlier news account stated that this section of track had maintenance performed on it recently.   That explains part of the cause, but not all of it.   Each train sends out a signal identifying itself – its location and speed that travels on the rails. – that’s how the train protection system knows what to do.  Either the lead train was not broadcasting, or there was an electrical break between the blocks.   Trains going through a block without being seen should have created “faults” in the monitoring system.  Probably they’re going to find the people who worked on the track disconnected the wiring for the safety system and didn’t reconnect it.

The track on the outside of the Metro tracks is not only for Amtrak, it is also Metro Rail, the commuter rail service.   If a Metro rail train was going around that curve on the outside track as the subway train was approaching, it could have obscured the moving train’s ability to see the stopped train.   Given it was rush hour, that seems very possible.  Time to check the train schedules for MetroRail…

Well, that idea doesn’t check out…   Maryland Transit  doesn’t run any contra-flow trains.   The run all of their cars into Union Station in the morning (and have to send 2 trains to Baltimore due to lack of track space in Union Station), and then send all the trains out in the evening.  But that track is also an active CSX freight line…  (Maryland Transit’s site mentions commuter trains are basically at the mercy of CSX’s freight movements)…  so it’s even more likely that a CSX train going inbound could have blocked her view.

WTOP is Reporting that Metro mangement has ordered the 1000 series cars to be used only in the middle of trains (trains are made up of groups of 2 cars – 2,4,6 or 8 cars).   By putting them in the middle, the newer trains are controlling the train set, and the 1000 series cars are not exposed to climbing in a collision.   They are also rechecking all 3,000 track circuits in the entire system for faults.
